TXM Disaster Relief Replacement Badge request

Every trained Texans on Mission Disaster Relief volunteer must have an identification badge. It helps other TXM volunteers, area officials and those we serve easily identify who you are.

To replace a lost or damaged badge, fill out the form below. In order to get a new badge, you must have an up-to-date TXM certification. Please allow 2-4 weeks processing time to receive your new badge.
